Our Experience: Three decades supporting complex conditions

For 31 years, Managing Director and Clinical Exercise Scientist David Trotter has guided people with complex health needs. His first client with multiple sclerosis (in 1994) set the course for a career focused on safe, specialised exercise for clinical populations. Before founding Reform Exercise Science, David operated Ormond Health and Fitness for 19 years, becoming known for injury rehabilitation and higher‑risk clients, including amputees, neurological and cardiac conditions, and post‑surgical recovery.

What our experience looks like in practice:

  • A team of six Master’s‑qualified exercise physiologists (EPs), all ESSA‑accredited, with ongoing first aid and CPR training.
  • Weekly clinical case meetings to review current research, refine protocols and problem‑solve complex presentations.
  • Long‑term, measured progression tailored to symptoms, fatigue and medical status.
  • Real‑world outcomes, such as an above‑knee amputee returning to competitive cycling, and a client with MS progressing from a 6 kg kettlebell to a 100 kg deadlift through careful dosage.

David has maintained continuous CPR certification since 1992, recently completing his 32nd recertification—reflecting our uncompromising approach to safety.

We practise within a robust clinical framework:

  • ESSA registration: adherence to professional standards, ethics and continuing education.
  • NDIS registration: comprehensive quality, safety and safeguarding compliance, with regular external audits.
  • Master’s‑level training: pathophysiology, pharmacology, behaviour change and exercise prescription for chronic disease.
  • Clear scope of practice: EPs are hands‑off allied health professionals who design and supervise exercise for chronic and complex conditions. We complement physiotherapy (which manages acute injury and hands‑on care), and we differ from personal training (which focuses on healthy populations and performance).

Research‑informed programming and dosage:

  • International and national guidelines inform our baselines (WHO, Australian Department of Health, Heart Foundation, AMA, ESSA).
  • Condition‑specific evidence shapes protocols—for example, MS fatigue management with low‑rep, longer‑rest strength work; cardiopulmonary monitoring for cardiac and respiratory conditions; and ultra‑conservative progression for chronic fatigue and long COVID to avoid post‑exertional malaise.
  • Precision in exercise dosage—intensity, volume, rest and frequency—is central. We monitor blood pressure, heart rate and oxygen saturation when indicated, and adjust in real time.

Our Authority: Trusted by doctors, hospitals and funding bodies

We are recognised within healthcare and government systems for safe, effective delivery:

  • Medical referrals: regular GP referrals via Chronic Disease Management Plans; referrals from specialists (orthopaedics, cardiology, neurology), allied health providers, hospitals including Alfred and Monash, local council programmes and NDIS support coordinators.
  • Medicare provider: eligible clients can access rebates for individual exercise physiology consultations, and we deliver the Type 2 Diabetes Group Service (one assessment and eight group sessions). Rebates are subject to Medicare schedules—your GP can advise on current rates.
  • NDIS registered provider: police checks for all staff, facility audits, accessibility compliance, insurance, quality systems and three‑yearly audits.
  • Professional education: David provides training to physiotherapy clinics on referral pathways and the role of clinical exercise in chronic disease management, contributing to sector capability and continuity of care.

Purpose‑built, accessible facility:

  • Level access throughout with ramps where needed, wide doorways, accessible bathroom, and dedicated disability parking at the entrance.
  • 100 metres to Moorabbin train station; bus stops and two‑hour parking nearby.

Equipment for every ability:

  • 15 pieces of cardiovascular equipment (including recumbent bikes for balance issues), pin‑loaded machines for seated training, free weights, balance tools and functional equipment—allowing full‑body training even when seated, and progression as capacity improves.

Quiet, focused environment and analogue accessibility:

  • No background music to support hearing, cognition and sensory needs; space for detailed clinical conversations during sessions.
  • Large‑print paper programme cards for clarity, notes and day‑to‑day adjustments—removing technology barriers for older adults.

Close supervision and emergency readiness:

    • Small‑group “supervised exercise physiology” sessions with two to three EPs on the floor; typical ratios around 8:1.
    • On‑site defibrillator; all staff current in CPR and first aid, with protocols for cardiac events, hypoglycaemia, falls, seizures and asthma.
    • Incident reporting and continuous improvement through our secure practice management system.

Transparent, affordable access:

  • Pay‑as‑you‑go—no contracts or upfront fees. HICAPS on site for immediate private health rebates where applicable.
  • Clear communication with GPs and specialists via assessment letters, progress reports and outcome measures.

How we work

A clear, supportive pathway helps you start safely and progress confidently:

  • Complimentary initial consultation (15–20 minutes): discuss health history, goals, funding pathways (Medicare, NDIS, DVA, My Aged Care), tour the clinic and meet the team.
  • Comprehensive initial assessment (45–60 minutes): medical review, functional testing and risk stratification to determine the safest starting point.
  • Programme design session (45 minutes): your individualised paper programme (6–10 exercises) with technique coaching, safety cues and home guidance if appropriate.
  • Ongoing supervised sessions: tailored progressions, symptom monitoring, and real‑time adjustments based on pain, fatigue, sleep and medication changes.
  • Regular reassessment (every 6–12 weeks): compare results to baseline, update goals and refresh your programme.
